Expanding the reach of healthcare beyond the office walls, SpringCreek Fertility has launched an online Patient Portal, allowing our providers to engage patients by providing anytime access to educational information, online bill pay, and much more, including:

View and download your medical records and securely send documents to another provider.
ART CALENDAR
Keep track of your cycle. View and print the most up to date instructions for each cycle day from your clinic.
APPOINTMENT Requests
Request to schedule or cancel your appointments, and view details of your past and future appointments.
ONLINE FORMS
View and fill out the necessary forms prior to your appointment.
Patients can access and update their secure Patient Health Record (PHR) from anywhere they have internet access, on mobile devices and home computers. Registered users can access their information here. The app can be downloaded to your mobile device from the Google Play Store or the IOS Apple Store.
